Moxie Notes

From Moxie

I have not personally done Cosmelan, but I made my mother do it once. It helped her melasma and uneven skin tone, but the downtime was tough - about 10 days. My issue is that if you have sensitive skin or even a little sun exposure, melasma can come back. This does not replace skincare.

Personally, I would rather use gentler peels or peel pads more consistently, strict sunscreen and pigmentation-focused ingredients.

Start Here

First, try this if

- Stubborn melasma or pigmentation under dermatologist guidance. - People able to follow strict at-home maintenance. - Patients who can avoid sun and heat during recovery. - Fitzpatrick III-VI only with pigment-experienced practitioners.

See a dermatologist first if

- Sensitive or reactive skin without careful assessment. - Poor sunscreen discipline. - Anyone expecting permanent melasma removal. - People unable to tolerate 7-10+ days of visible downtime.

What It Is

Cosmelan is a professional depigmentation peel system commonly used for melasma and stubborn pigmentation. It usually includes an in-clinic mask and a strict at-home maintenance protocol. It is not a simple glow peel.

Moxie covers it because she saw it help her mother, but she personally does not like the downtime and recurrence reality.

How It Works

Cosmelan works by targeting melanogenesis pathways and exfoliating pigmented surface layers through a structured depigmentation protocol. The goal is to reduce visible pigment and regulate ongoing melanin overproduction. Melasma is chronic and relapse-prone.

That means the peel may improve pigment, but it does not remove the underlying tendency. Sun, heat, hormones and irritation can bring pigment back.

The Evidence

Evidence: Moderate
GradeB — Moderate evidence

Concerns Addressed

PIH, melasma, sun damage and dullness/uneven tone.

Chemical peel approaches in melasma show good evidence when combined with topical therapy. Consensus review literature notes that melasma clearance can be better and faster when chemical peels are combined with topical therapy, while PIH risk in skin of color remains important. The evidence supports combination care, not peel-only thinking.

Source

Brand-specific Cosmelan RCT data was not located in this search; the broader evidence for the underlying category (TCA/combination depigmenting peels for melasma) is solid — a systematic review of melasma chemical peels (24 studies, 15 RCTs, n=1,075) found glycolic acid the most consistently safe/effective, with a 15% TCA peel also producing significant MASI score reduction. Grade reflects moderate category-level evidence with a caveat that brand-specific (Cosmelan) trial data is thinner — same caveat pattern previously applied to Jalupro.

Am I a Good Candidate?

- Stubborn melasma or pigmentation under dermatologist guidance. - People able to follow strict at-home maintenance. - Patients who can avoid sun and heat during recovery. - Fitzpatrick III-VI only with pigment-experienced practitioners.

Caution or Avoid

- Sensitive or reactive skin without careful assessment. - Poor sunscreen discipline. - Anyone expecting permanent melasma removal. - People unable to tolerate 7-10+ days of visible downtime.

What to Expect

Step 1 - Consultation: Pigment type, melasma triggers and skin sensitivity are assessed. Step 2 - Preparation: Skin may be prepped; active routines may be adjusted. Step 3 - The procedure: An in-clinic depigmentation mask is applied and removed after instructed timing.

Step 4 - Immediately after: Redness, peeling, burning, tightness and visible downtime can occur. Step 5 - Timeline and maintenance: Results depend heavily on the home protocol and long-term sun protection.

Cost Breakdown

Frequently Asked Questions

Does Cosmelan permanently remove melasma?

No — it may improve visible melasma when paired with strict at-home maintenance, but melasma is chronic and relapse-prone. Sun, heat, hormones and irritation can bring pigment back even after a successful treatment.

How much downtime does Cosmelan require?

Significant — Moxie observed about 10 days of tough downtime when her mother had the treatment, including redness, peeling, burning and tightness.

Has Moxie personally tried Cosmelan?

No — she had her mother do it, and it helped her mother's melasma and uneven tone. Moxie personally prefers gentler, more consistent peels and strict sunscreen over the heavier downtime of Cosmelan.

The Verdict

Not For Me

Strong result, heavy downtime.
